Insurance Information

Family Vision Center works with many major health insurance carriers and vision benefit plans.

Dr. Guy is a participating in-network provider for all major insurances, including:

  • Excellus BCBS
  • Blue Cross Blue Shield
  • MVP
  • Aetna
  • Cigna
  • Medicare
  • United Health Care
  • Vision Service Plan (VSP)
  • MetLife

Please note that we accept Blue Choice Option, Family Health Plus, Child Health Plus, and MVP Option. We do not accept straight Medicaid or Benefit card.

We also offer self-pay options for eye care and eyewear if you do not have insurance, or if your insurance is out-of-network with our office. Self-pay options will be explained to you upon request.

Our staff will be happy to answer any questions about your health insurance coverage to the best of their ability. However, please keep in mind that because the health insurance landscape is constantly changing, we don’t always have access or the means to obtain your coverage information. We encourage you to contact your health insurance company directly and obtain details about your vision benefits so that you are aware of what your plan covers.
